On Monday, January 6, 2025, Firefighter 1 Jeffrey Fiala responded to a residential fire. While conducting a search and rescue of the two-story apartment complex, he became separated from his fellow firefighters and a mayday was called. He was rescued from the structure and transported to the emergency room where he was pronounced deceased. Investigation into the incident continues.

HONOLULU — A 25-year-old Honolulu firefighter has died, and several others were hospitalized during a three-alarm fire and roof collapse.
Firefighters responded at 7:42 p.m. on Jan. 6 to a two-story building in the McCully neighborhood, Island News reported. A mayday was issued for two missing firefighters during the search. One escaped, while the other was rescued after a roof collapse, said Captain Jamie Song.

Four firefighters and one civilian are in serious condition, KHON2 reported. The fire was controlled by 9:07 p.m. and fully extinguished by 11:31 p.m. The cause of the fire and death are under investigation.
